The easiest way of determining that you are getting an appropriate template is to compare it with one of your old doctor's notes. If they are similar, you can rest assured that no one will be able to tell its fake.


2. As far as the information written on the doctor's note is concerned, every doctor's note needs to contain four things in order for it to pass off as authentic. The first is the name and contact details of your doctor, the second is your name and contact details, the third is the date on which you visited the doctor and the last is a description of the consultation along with the doctor's medical advice.
